The requirements for health policy frameworks will change due to the increasing number of people with chronic illnesses. Health policy follows national development trajectories, from which health policy reform perspectives can and must be considered. These are illustrated using German health policy as an example. Since World War II, health policy has been shaped by four phases: consolidation, expansion of social security, the phase of cost-containment policy, German reunification including East German convergence, and finally, market regulation policy. The latter is evaluated with regard to its impact on healthcare provision, and requirements for a future-oriented health policy are outlined. Health policy must establish the framework to ensure that healthcare providers collaborate in care networks for the benefit of those affected.
