The term digitalization is experiencing inflationary use. In this context, opportunities and risks are mostly unclear, yet digitalization is still attributed with high potential. In the context of chronic care, digitalization affects a multitude of care-providing organizations as well as health professionals. Human vs. technology—but why not better engage people as partners for a strategic realignment? This article examines the dimensions of people, process, strategy, and technology from the perspective of organizations. It questions to what extent stronger management of cross-organizational care processes is needed and whether digitalization can help precisely at this point to deliver the right information at the right time, to the right place, and in the right form.
