Chronically ill individuals require cross-setting, needs-based care. This article outlines, primarily from a systems theory perspective, key characteristics of the healthcare system that give rise to inherent connectivity issues between autonomously and specialized operating settings. It concludes with suggestions for actor-related approaches by health and non-health professionals to mitigate these barriers.
