Chronic care refers to the comprehensive provision of services for individuals with ongoing physical, mental, or emotional impairments and risks that may result in a long-term need for help and support. In this context, chronic care encompasses disease prevention, treatment, health maintenance and restoration, everyday support and care, as well as ensuring participation and (re-)inclusion. The understanding of what constitutes a chronic illness is not clear-cut, but rather can be determined by characteristics that vary in significance in individual cases. To situate chronic care in practice, the conditions of the care setting must be taken into account. At the same time, the design of the respective person-centered care arrangements is a key focus. The latter includes the professional and non-professional actors involved in care, the objectives of care, and, ultimately, care planning.
