After discussing the importance of a shared orientation model for chronic care, the Trajectory Work Model (TWM), with its subjective patient perspective and its structurally interactive understanding of processes, is presented as common ground for a coordinated care practice. Furthermore, the fundamental understanding of chronicity, the nature of the coping tasks to be performed, and the resulting phase progressions frame the actor-related design options for the substantive negotiation of professional boundary work.
