Spieghel hernias are rare hernias caused by a congenital or acquired defect in the semilunar line. They are often difficult to recognize due to their location and atypical appearance. These rare hernias are associated with a high risk of incarceration and strangulation and should be operated on as soon as possible after diagnosis. Open and laparoscopic techniques are well described in the literature. Since 2015, robot-assisted treatment of Spieghel hernias has also been reported in the literature. The advantages of robot-assisted hernia repair include better visualization of the anterior abdominal wall, better ergonomics, greater dexterity and higher precision compared to laparoscopic techniques. Results show less postoperative pain and faster recovery time compared to laparoscopic procedures. This chapter reviews the history of Spieghel hernia and describes the surgical planning and technique of robotic-assisted Spieghel hernia repair.
