A typical Sunday morning in an emergency department (ED) with plenty of people to care for who were still there from the night before. As always in the morning, the handover of patients from the night shift to the day shift took place. Among them was Mr. W., a cardiology patient being just over 80 years old. He had called the Emergency Medical Service (EMS) in the middle of the night after waking up with burning pain in the left thoracic region. The EMS physician gave him 500 mg of acetylsalicylic acid, 5000 IU of heparin, and 2 puffs of nitro on suspicion of an acute coronary syndrome (ACS), given his known 3-vessel coronary heart disease with two implanted stents.
