The most important forms of orthostatic hypotension (OH) are sympathicotonic OH, autonomic-neurogenic (asympathicotonic) OH, and drug-induced OH. A special form of orthostatic intolerance is POTS (“postural orthostatic tachycardia syndrome”) with inadequate tachycardia, but without significant blood pressure drop when standing. Often, additional non-cardiovascular symptoms occur. The most important diagnostic measure for OH is the standing test according to Schellong, in individual cases a tilt table examination. In the case of asympathicotonic OH, tests of autonomic nervous function are to consider. For sympathicotonic forms, non-drug measures are predominantly in the foreground; for autonomic-neurogenic forms, different approaches of a mostly only limitedly effective, drug therapy are added.
