Module 4: Cognitive Reappraisal

According to the functional model of somatoform disorders, patients’ evaluations and thoughts about their somatic symptoms play a central role in the development and maintenance of symptoms. As a rule, working on cognitions requires a lot of time and is rather scarce with five sessions in the present module. Using a behavioral experiment and guided discovery strategies, patients are first introduced to the role of thoughts in relation to their somatic symptoms and to the ABC model. In the second session of the module, the exploration of the patient’s individual less helpful thoughts is front and center. The third and fourth sessions of the module focus on the modification of cognitions. In particular, symptom attributions are challenged through symptom provocation exercises in the form of behavioral experiments. In addition, less helpful thinking patterns typical of somatoform disorders are addressed and cognitive strategies are applied with which these patterns can be modified. At the end of the cognitive module, it should not be forgotten to explore and question the patient’s concept of health.
