Vulvar dystrophy is an outdated term formerly encompassing chronic nonneoplastic vulvar disorders such as lichen sclerosus, lichen simplex chronicus, and mixed forms. Contemporary ISSVD classification replaces this terminology with specific diagnoses including LS, LSC, and lichen planus. LS predominantly affects postmenopausal women and carries a 4–5% risk of squamous cell carcinoma, whereas LP has a lower malignant potential and LSC is benign. The LS–LSC overlap typically occurs in middle-aged to postmenopausal women due to chronic scratching superimposed on LS. Accurate diagnosis, vigilant surveillance, and targeted corticosteroid therapy remain central to management and malignancy prevention.
