For certain types of rhegmatogenous retinal detachment, scleral buckling—when appropriately performed—sometimes offers distinct advantages in terms of efficacy and cost-effectiveness. This chapter will discuss why scleral buckling has become less popular than in the past and introduce the rationale for choosing it as a first-line option. The aim is to help young retinal surgeons understand that scleral buckling is straightforward and readily mastered.
