Preterm birth (PTB) remains one of the most significant challenges in obstetrics, accounting for a large proportion of neonatal morbidity and mortality globally. Cervical insufficiency, diagnosed as a short cervical length (CL) measured on ultrasound, is a well-established risk factor [1]. Over the past two decades, vaginal pessaries have emerged as a noninvasive intervention aimed at preventing PTB. However, despite widespread clinical interest, evidence supporting their effectiveness has evolved inconsistently [2].
