Patient Profile
摘要
The clinic’s patient profile for 2023 should reflect the type and origin of its clients. Which specialist groups referred the patients? What were the main and secondary mental and somatic diagnoses? How are they classified according to the Western ICD-10 system and traditional Chinese medicine? Further details of the evaluation include the chronicity of symptoms, patients’ age and gender, their medical history, their use of previous medical and psychological services, and the length of their sick leave. The profile also records patients’ subjective attitudes towards the potential causes of their symptoms, and their personal commitment to improving their health-related behaviour and coping with illness. The IHM lifestyle programme and psychoeducation focus on basic psychosocial and somatic skills and functions. To identify patients at the highest risk of cardiovascular disease, the 10-year overall risk of cardiovascular events was calculated using the SCORE-2 method. Exercise, diet and sleep are now integral to TCM-hospital care. This is important because people with mental health conditions are more likely to develop physical health problems and have a shorter life expectancy. Physical exercise has a positive impact on patients’ mental health. This chapter explains the concept of indicators and questionnaires and presents the results of various analyses of psychosocial and somatic risk and protective factors within the three categories of the health traffic light for the current reporting year. Coaches and patients use the patient profile to evaluate and plan therapy, as well as to manage lifestyles.
