Prodromes of Dementia: Mild Cognitive Impairment, Biomarkers, and Genetics

There have been remarkable advances in the field of dementia diagnostics and care over time. Within the span of my own career over the last 30 years, medications (i.e., cholinesterase inhibitors and NMDA receptor antagonists), as well as identification of genetic risk markers and biomarker indices, have emerged for use within clinical practice. Such discoveries have certainly impacted approaches to clinical care, although there remains no cure for dementia. Further, prediction of who will contract various forms of dementia remains elusive. This chapter will review the current state of prodrome identification and prediction, including review of mild cognitive impairment (MCI). Discussion of ethical factors associated with biomarker and genetic marker testing in this group will also be discussed.
