A 68-year-old man underwent an open repair of a juxtarenal abdominal aortic aneurysm (AAA) in 2018. Following open repair, on routine surveillance, he was found to have a 5.7 cm pararenal AAA in May 2024. He underwent deployment of a physician-modified endograft with four-vessel fenestration using percutaneous access. Postoperatively, the patient was found to have a left retroperitoneal hematoma from guidewire puncture of a branch of the left renal artery, which resolved spontaneously. A follow-up CTA of the abdomen and pelvis 6 months later showed satisfactory exclusion of the aneurysm without any endoleak.
