Post-radiation Subclavian-Axillary Occlusion with Severe Hand Ischemia

A 74-year-old woman presented with severe ischemia of the right upper extremity due to post-radiation-induced subclavian-axillary artery occlusion. An unsuccessful endovascular attempt for revascularization of the subclavian-axillary artery required an interposition graft placement. Graft occlusion occurred 26 months later. A crossover axillo-axillary graft was performed. Recurrent ischemia of the right hand required three additional operations (revision of distal anastomosis), graft thrombectomy twice, and a vein bypass to the distal portion of the brachial artery. The patient died 9 years following the index procedure from carcinoma of the lung with satisfactory arterial flow to both upper extremities.
