A 56-year-old man underwent left femoral to mid anterior tibial artery bypass using non-reversed spliced veins with contralateral great saphenous vein, basilic, and cephalic veins. The distal segment of the spliced vein graft was brought through the interosseous membrane and anastomosed to the mid-anterior tibial artery. Patient had two prior failed femoral–infrageniculate bypass grafts. At a follow-up, the patient underwent angioplasty twice for distal anastomotic stenosis. He had satisfactory reconstruction by clinical exam and duplex imaging at 24 months. However, the spliced bypass graft occluded 30 months after its performance without deterioration in symptoms of lower extremity ischemia.
