Deep Vein Arterialization for Severe Infrapopliteal Artery Occlusive Disease with Foot Gangrene

Deep vein arterialization (DVA) using the LimFlow system was performed in a 67-year-old man with gangrene of the right great toe with contralateral below-knee amputation. He required open transmetatarsal amputation (TMA). Reintervention after 3 months and 8 months for donor tibial arterial inflow stenosis and popliteal artery stenosis resulting in diminished flow volumes and thrombosis of the stent graft, respectively, was necessary to maintain stent patency, continued documented pedal venous arterialization, and progressive wound granulation. Although the LimFlow stent graft thrombosed 1 year after placement, the transmetatarsal amputation wound ultimately healed.
