A 68-year-old man presented with a pulsatile mass in the neck. He had a history of right carotid endarterectomy in 2005. Repeat carotid endarterectomy with patch graft was performed in 2020. CTA of the neck and brain showed a right carotid aneurysm measuring 2.7 × 2.5 cm involving the right common carotid artery (CCA) and proximal internal carotid artery (ICA). The patient underwent resection of a carotid aneurysm with an interposition of a rifampicin-soaked PTFE graft with a satisfactory outcome.
