Simultaneous Standard Recovery of the Lungs and Abdominal Normothermic Regional Perfusion for Recovery of the Abdominal Organs in Controlled DCD

The simultaneous retrieval of lung and abdominal grafts using abdominal normothermic regional perfusion (A-NRP) in controlled donation after the circulatory determination of death (cDCDD) increases the complexity of the procurement procedure, and may harm the grafts due to differences in abdominal and lung temperature or to inadequate perfusion pressure in the pump due to bleeding into the chest after the heart-lung block is removed or after vena cava clamping. Some professionals are still reluctant to combine the cooling and rapid recovery (RR) technique in the lungs with A-NRP for abdominal grafts. However, this combined method offers many advantages for abdominal grafts and seems to be safe in lung grafts, achieving outcomes equivalent to those reported with donation after the neurological determination of death (DNDD) grafts.
