Capsulorrhexis

Capsulorrhexis is the step immediately following the main incision in cataract surgery, wherein the surgeon opens the anterior capsule and gains access to the lens cortex and nucleus for phacoemulsification. The resulting opening is also called a capsulorrhexis or rhexis. Femtolaser and thermoablative devices are sometimes used in advanced eye care centers, but most surgeons use a manual capsulorrhexis either by tearing or cutting the anterior capsule (Basic principles of ophthalmic surgery. American Academy of Ophthalmology; 2024). Regardless of the modality, errors during the capsulorrhexis are often irreversible and may impact the remaining course of surgery. The instructions below will teach a few manual capsulorrhexis techniques and describe when they are appropriate.
