Surgery has long embodied the union of science, skill, and innovation. From ancient trepanations and early suturing techniques to modern robotic-assisted and artificial intelligence-guided procedures, surgical practice reflects a continuous evolution driven by necessity and ingenuity. This chapter traces key milestones in surgical innovation across history, beginning with early pioneers such as Sushruta, Hippocrates, and Galen, whose foundational work in wound care, reconstruction, and endoscopy laid the groundwork for modern operative medicine. The transformative introduction of anesthesia and antiseptic technique in the nineteenth century marked a turning point, shifting surgery from a perilous last resort to a scientifically rigorous discipline. Subsequent advances demonstrate how technology continues to expand surgical capability. The chapter concludes by examining parallel innovations in surgical education, including competency-based curricula, telesimulation, and virtual reality training.
