Primary care systems worldwide face challenges from the rising burden of chronic diseases, persistent health inequities, workforce shortages, and evolving patient expectations. This chapter examines how innovations in primary care delivery are transforming the field toward more proactive, team-based, digitally integrated, and equity-centered models of care. Telehealth and improvements in electronic health records have reshaped how physicians and the healthcare teams can deliver continuous, data-driven care. We review the evolution of primary care physicians’ roles from episodic providers to coordinators of longitudinal, team-based care that addresses both clinical and social determinants of health. Key digital innovations including optimized electronic health records, clinical decision support systems, telehealth, remote patient monitoring, and patient portals are reshaping how care is delivered, enabling continuous monitoring and flexible access while reducing traditional barriers. The chapter highlights the critical importance of interprofessional collaboration, including team-based care models and integrated behavioral health, which improve care coordination, medication safety, and pediatric behavioral outcomes. Patient-centered medical homes, same-day appointments, and group medical visits are some of the innovations that have emphasized how focusing on equitable care has improved patient outcomes and satisfaction. Team-based care has also proved to be essential in addressing social determinants of health and coordinating care with different medical disciplines. Finally, these innovations call for continuous quality improvements, changes in policies, and an adaptable approach. The chapter highlights general practitioners as pivotal players in being advocates and educators for innovative changes in general practice, uniquely positioned to drive system-wide transformation toward more effective, equitable, and patient-centered primary care delivery.
