Iodine-123 (Half-Life 13.2 h; Decays by Electron Capture to Te-123) and Iodine-131 (8.1 d, Beta Minus Emission, Xe-131)

I-123 (159 keV) and I-131 (364 keV) are used to image thyroid disease along similar lines to those described under pertechnetate (Tc-99mO4). They have the additional role of imaging thyroid cancer, either in a diagnostic setting with I-123 and whole-body imaging for detection of metastases, or for therapy with I-131 in thyrotoxicosis or thyroid cancer. I-123 is complexed to ioflupane, which targets dopamine transporters in nerve terminals in the basal ganglia, for the diagnosis of Parkinson's disease and Parkinsonian syndromes, and to MIBG, which, in a similar fashion, targets noradrenaline transporters for the diagnosis of phaeochromocytoma, paraganglioma, neuroblastoma and a range of neuroendocrine tumours.
