HMPAO is a lipophilic chelator of Tc-99mO4. Following i.v. injection, Tc-99m-HMPAO is cleared from blood rapidly and becomes widely distributed in the body, freely entering cells on account of its lipophilicity. It is therefore an example of a chemical microsphere and was developed to image the distribution of cerebral blood flow. Later, its properties as a lipophilic chelator were exploited to label autologous leucocytes for white cell scanning. Tc-99m-HMPAO is used to diagnose cerebral atrophy in the dementias and to identify epileptogenic foci in epilepsy but has been largely replaced for brain imaging by FDG PET/CT.
