Accountable Care Organizations (ACOs) are entities that consist of healthcare providers who enter into an agreement to manage the care delivered to a discrete group of patients. ACOs facilitate the transition from volume to value by providing an organizational framework that places emphasis on collaborative approaches to healthcare and financial incentives to align provider behavior. A variety of Accountable Care Organization (ACO) models exist, each providing distinct levels of financial incentives that span from shared savings arrangements to full financial accountability for the comprehensive cost of care for a designated patient population. These financial incentives are unlocked once specific quality measures are achieved; the emphasis is placed on not withholding necessary healthcare services to achieve positive financial outcomes. Success in ACO participation is reliant on key competencies including but not limited to data management, financial accounting, and an understanding of ACO payment structures and governance. This chapter will discuss the history and current and future state of ACOs, and the roles anesthesiologists can play as ACO participants in delivering high quality, cost effective, and collaborative care.
