This case describes a 14-year follow-up of a patient implanted with His bundle pacing (HBP) for third-degree atrioventricular block and right bundle branch block (RBBB). HBP maintained ventricular synchrony with a narrow paced QRS and preserved left ventricular ejection fraction over time, despite 100% ventricular pacing. Electrical parameters remained stable, and His potentials were still identifiable 14 years post-implant. This long-term observation highlights the durability, stability, and physiological benefits of HBP in preventing pacing-induced cardiomyopathy through sustained conduction system capture.
