His Bundle Pacing in First-Degree AV Block

His bundle pacing (HBP) represents a physiological approach for patients with first-degree atrioventricular (AV) block and a narrow QRS, allowing PR interval normalization while maintaining native ventricular activation. This case illustrates the value of unipolar mapping in identifying distal His potentials and the importance of differentiating selective from non-selective His capture during threshold testing. The procedure, largely guided by intracardiac electrograms with minimal fluoroscopy, demonstrates that HBP can achieve precise conduction system recruitment and restore AV synchrony without compromising QRS morphology.
