Three-dimensional electroanatomical mapping (3D-EAM) enables precise localization of the His bundle and facilitates conduction system pacing (CSP) with minimal fluoroscopy. In this patient with left bundle branch block (LBBB) and mildly reduced left ventricular function, 3D-EAM allowed accurate identification of the His cloud and guided targeted lead placement using a deflectable sheath with integrated mapping capability. The procedure achieved complete LBBB correction, restored ventricular synchrony, and resulted in a narrow QRS with excellent electrical parameters. This case illustrates how EAM-guided His bundle pacing enhances accuracy, reduces radiation exposure, and confirms resynchronization through left ventricular activation mapping.
