The rationale for correcting bundle branch block through conduction system pacing relies on the proximal location of the majority of bundle branch blocks (Tung & Upadhyay, Arrhythm Electrophysiol Rev 9:28–33, 2020). The principle also applies to the correction of left bundle branch block via left bundle branch pacing. This chapter reports a case of LBBAP using a stylet-driven lead in a patient with baseline LBBB and a QRS duration of 160 ms, performed as part of an ablate-and-pace strategy.
