Septal perforation—an acquired or congenital continuity defect of the nasal septum—presents a unique reconstructive challenge because successful closure must restore both structural integrity and nasal airflow physiology while minimizing morbidity. This chapter reviews surgical approaches to septal perforation repair using a variety of different intranasal flaps.
