Pain and Itch Control in HS (Clinical Cases in HS)

Hidradenitis suppurativa has the worst quality of life (QoL) of any dermatologic disease (Patel et al. Curr Pain Headache Rep 21:49, 2017; Li et al. Dermatol Rev 3:137–49, 2022; Wolk et al. Br J Dermatol 183:999–1010, 2020; Matusiak et al. Acta Derm Venereol 98:191, 2018; Orenstein et al. Br J Dermatol 188:41–51, 2022; Savage et al. J Am Acad Dermatol 85:187–99, 2021; Molina-Leyva and Cuenca-Barrales Dermatology 236:59–65, 2020; Matusiak et al. Acta Derm Venereol 90:264–8, 2010). Over 60% of HS patients report severe QoL impairment and identify pain as their most impactful symptom (Matusiak et al. Acta Derm Venereol 98:191, 2018; Savage et al. J Am Acad Dermatol 85:187–99, 2021; Matusiak et al. Acta Derm Venereol 90:264–8, 2010). This chapter reviews current understanding of pain mechanisms in HS and highlights the underrecognized clinical importance of itch. We describe available tools for assessing pain and itch and summarize current treatment guidelines for the management of HS-related pain.
