Osteochondral lesions of the talus (OLTs) are lesions of the articular cartilage and the underlying subchondral bone. These lesions often originate after a traumatic event and may cause persistent complaints such as deep ankle pain. Lesions should be thoroughly assessed both clinically and radiologically. Treatment options depend on the lesion characteristics. Nonsurgical treatment is successful in approximately four out of ten patients. If non-operative treatment fails, surgical treatment using both arthroscopic and open techniques is possible, which are discussed in this chapter.
