Extraction of the Perinatal Brain and Spinal Cord During Necropsy

The fragile nature of the premature fetal brain makes it an absolute necessity to perform brain extraction as well as spinal cord removal with utmost care. The technical difficulties that come with handling of the premature brain can be mitigated by underwater dissection, weighing in fixative, use of a hardening fixative, and where practical, removal of the spinal column with the spinal cord in situ, for sectioning after fixation and decalcification. Removal of encephaloceles and myeloceles with the brain and surrounding inact skin en bloc is recommended. Hardening fixation for the preterm brain are encouraged: formalin with 5% glacial acetic acid by volume or Bouin's fluid are suitable. Some colleagues have also used a combination of formalin and zinc sulfate.
