In the United States, all graduate medical education programs—residencies and fellowships—and the institutions that sponsor them are overseen by the Accreditation Council for Graduate Medical Education (ACGME). This chapter describes the process involved, including obtaining and maintaining accreditation. First, we review the ACGME requirements for a training program, including the structure and process for evaluating trainees and milestones. We then describe how the ACGME annually reviews programs and the possible outcomes of that evaluation. Finally, we offer advice for a training program seeking new or continued accreditation.
