Change is a constant in graduate medical education (GME) programs and in academic departments. Multiple circumstances require program directors to nimbly adapt to conditions outside of their control. Much of the stress incurred in leading a residency or fellowship program is the need to react to such events. Changes may be internally driven and planned carefully, or externally imposed and abrupt. Each variety requires coordinated effort from the department to ensure that the missions of training and patient care are not disrupted. This chapter describes the sudden change managed by the Hahnemann Hospital psychiatry residency program as it faced imminent closure. The event is considered from the perspectives of a department chair, a vice chair/program director, a chief resident, and an incoming first year trainee. Reflections and recommendations are provided on navigating communication, maintaining support systems and self-care, and navigating career changes during a disruptive time of residency closure as examples of strategies that may be useful during any major change sustained by a GME program.
