Acute Type B aortic dissection (aTBAD) remains a potentially lethal diagnosis, requiring thorough understanding, rapid diagnosis, and treatment. Thoracic endovascular aortic repair (TEVAR) has revolutionized the treatment of complicated aTBAD; however, important areas of active investigation remain regarding techniques, devices, and extent of coverage. Furthermore, the role of TEVAR for uncomplicated aTBAD has yet to be defined. Endovascular therapy for chronic Type B aortic dissection (cTBAD) is feasible, but has many more challenges, and open aortic replacement continues to be the optimal therapy for patients who can tolerate the procedure.
