Somatic Symptom Disorder and Illness Anxiety Disorder

This chapter reviews current diagnostic criteria for Somatic Symptoms Disorder (SSD) and Illness Anxiety Disorder (IAD). SSD/IAD prevalence, risk factors and disparities are overviewed. Chapter presents three brief screening tools: Patient Health Questionnaire 15 item scale (PHQ-15), Somatic Symptom Scale-8 (SSS-8), and Short Health Anxiety Inventory (SHAI-14) and outlines screeners’ practice-relevant characteristics. The non-pharmacological conceptualization section includes three key “messages” which physicians need to articulate to effectively partner with patients suffering from SSD/IAD. Additionally, three challenges faced by patients with SSD/IAD (catastrophizing, behavioral avoidance/safety behaviors, difficulties with tolerating strong negative emotions/uncertainty) are outlined and interventions to address these concerns are presented. Integrative medicine section introduces mind-body practices (MBPs). A case vignette is used to illustrate the presented concepts.
