Endovascular Embolization of Cerebral Aneurysms and Arteriovenous Malformations

Endovascular intervention has become an increasingly popular option for the treatment of cerebrovascular pathology. With advances in catheter technology and embolization techniques, a minimally invasive and safer option can be offered to patients via an endovascular approach. Endovascular treatment of cerebral aneurysms typically involves intrasaccular embolization with coils assisted by stenting across the neck of the aneurysm or endoluminal embolization using flow diverters. Cerebral vascular malformations are typically treated with liquid embolics with the intent of cure or as an adjunct to other treatments. Regardless of pathology or choice of technique for embolization, endovascular intervention of cerebrovascular pathology requires coordination and planning involving neurointerventionalists, anesthesiologists, and the neuromonitoring team to mitigate risks and reduce complications during the procedure. In this chapter, we will review setup and planning for endovascular intervention, present details of treatment options and techniques for cerebral aneurysms and vascular malformations, and discuss the role of neuromonitoring in these procedures.
