Traditionally, the first surgical procedure to enlarge the vault in an infant with Apert syndrome was the fronto-orbital advancement. In 2009, teams from two large craniofacial centers located in Philadelphia (USA) and Rotterdam (Netherlands) both switched to a routine posterior vault expansion with distraction. The outcomes of this procedure have significantly improved—with fewer reoperations and a lower prevalence of raised intracranial pressure and/or Chiari malformation during follow-up—while the rate of complications has remained low.
