Osteocutaneous Scapular Flaps

Scapular bone has been well established as a safe reliable donor site for vascularized bone flaps with minimal and acceptable donor site morbidity. The anatomy is consistent with two segments of bone that can be utilized based on the circumflex scapular artery or the angular artery to harvest the lateral scapula and scapular tip. Additional tissue including muscle, fascia, and skin can be harvested to create chimeric flaps based on the subscapular system. VSP can be utilized to perform presurgical planning. Scapular bone flaps should be considered in complex cases that require multiple tissue types and/or when the fibula flap is contraindicated. The parascapular fasciocutaneous flap is an excellent donor site with a relatively inconspicuous scar so it should be routinely considered as an alternative to the ALT flap.
