The Balmis Expedition (1803–1806), led by Francisco Javier Balmis, was a pioneering global public health initiative to distribute the smallpox vaccine to Spanish overseas territories. Sponsored by King Carlos IV, the mission aimed to vaccinate whole populations, train local health workers, and establish vaccination boards to ensure long-term immunization efforts. The expedition used a chain of arm-to-arm inoculations, relying on orphaned children to maintain the vaccine’s efficacy during transoceanic travel. Divided into two groups, Balmis covered North America, the Philippines, and China, while José Salvany focused on South America. Despite logistical challenges, the expedition laid the foundation for modern vaccination campaigns and contributed significantly to the eventual eradication of smallpox.
