Kidney transplants significantly improve and prolong patients’ lives, especially for those with advanced chronic kidney disease (CKD), which often progresses silently. In Colombia, 1,190 kidney transplants were performed in 2022, yet over 3,600 people remain on the waiting list—90.8% of whom need a kidney, making it the country’s most requested organ. Pre- and post-transplant care is crucial for patient well-being and transplant success, requiring patients to monitor their health and adhere to their treatment regimen actively. This study presents Renatus, a system designed to support renal transplant patients by enabling the timely monitoring of key health indicators, including weight, blood pressure, temperature, fluid balance, and medication intake. Renatus integrates sensors and a user-friendly interface for patients to record and store data, which healthcare professionals can later review. An MVP (Minimum Viable Product) was presented to a focus group of users to assess satisfaction and perceived usefulness. Results showed positive feedback, highlighting Renatus’ potential to support pre- and post-transplant care. While this work provides an initial exploration of the system’s feasibility, it does not yet reflect full-scale implementation in the healthcare system. Future development will incorporate user feedback to refine and expand the system, as well as Electronic Clinical History integration, thereby enhancing patient autonomy and improving health outcomes.
