The Evolution of Cardiovascular-Kidney-Metabolic Syndrome

This chapter focuses on the evolution of cardiovascular-renal-metabolic syndrome from its two initial components (cardio-renal syndrome and cardio-metabolic syndrome) and describes how these conditions converged. Cardio-renal syndrome refers to the interplay between the heart and the kidneys where the dysfunction of one system leads to the dysfunction of the other. Based on the primary insult site (cardiac and renal) and the nature of the dysfunction (acute or chronic), it has been classified into five subtypes. Both reduced cardiac output and increased central venous perfusion are important pathophysiological mechanisms and treatment targets. Cardio-metabolic syndrome represents a chronic and progressive condition marked by interconnected metabolic abnormalities that collectively increase the risk of atherosclerotic cardiovascular disease and type 2 diabetes mellitus. It represents a distinct clinical entity with its own pathophysiology in which dysfunctional adipose tissue, insulin resistance, dyslipidemia, chronic inflammation, and prothrombotic factors play important roles. The interplay between these two entities was formally defined by the American Heart Association in 2023. It has been divided into four stages, ranging from healthy individuals in whom management should focus on prevention to symptomatic cardiovascular disease and kidney disease. These collaborative frameworks synergize expertise across cardiology, nephrology, and endocrinology to optimize patient outcomes through comprehensive therapeutic strategies.
