Migraine is a leading cause of neurologic disability worldwide, with the neuropeptide calcitonin gene-related peptide (CGRP) found to play a pivotal role in migraine pathophysiology. Eptinezumab is a humanized monoclonal antibody targeting the CGRP ligand. Clinical trials, including the PROMISE-1, PROMISE-2, PREVAIL, and DELIVER trials, have demonstrated efficacy and safety in treating episodic and chronic migraine, including migraines refractory to other agents. In the current landscape of CGRP therapies, Eptinezumab is differentiated by its ability to be delivered intravenously with rapid onset and a long half-life that allows for quarterly dosing. Altogether, Eptinezumab is a valuable tool in the migraine therapeutic armamentarium; however, high cost and payer restrictions remain a barrier to access.
