Epicrania Fugax

Epicrania Fugax is a rare primary headache disorder that is likely to be undiagnosed due to its peculiar clinical presentation. We report the case of a 61-year-old female who experienced recurrent paroxysms of dull, pressure-like pain localized to the right side of her head. These paroxysms progressed rapidly from the frontal to the occipital region, were short in duration, and occurred up to 20 times daily, with some episodes lasting for two consecutive weeks. The patient denied any identifiable triggers, migrainous features, or associated autonomic symptoms. Laboratory tests, brain and cervical magnetic resonance imaging (MRI), and temporal artery Doppler ultrasound yielded no significant findings. Following unsuccessful attempts at titrating antidepressant medications and optimizing anti-inflammatory treatments, the patient was diagnosed with Epicrania Fugax. She responded favorably to treatment with gabaergic medications, leading to significant symptom relief.
