Psychogenic Nonepileptic Seizures and Comorbid Epilepsy in Youth

Psychogenic nonepileptic seizures (PNES) in youth are associated with high rates of psychiatric and medical comorbidities, including epilepsy. Early PNES diagnosis leads to timely psychiatric treatment and improved outcomes. The gold standard approach to diagnosis includes a comprehensive history, psychiatric assessment, and a video EEG recording capturing the PNES event without an ictal EEG epileptiform correlate. PNES treatment is multidisciplinary and includes a team of clinicians with the mental health clinician taking the primary role, a neurologist/epileptologist, the patient’s primary care provider, school staff, and family. The treatment strategy for patients with PNES and comorbid epilepsy must address both disorders. This chapter includes a case discussion of a teenage girl with PNES and epilepsy.
