Anterior Dural (Hofmann’s) Ligaments

The anterior dural ligaments are lesser known structures that provide stability to the spinal cord by tethering the dura to the spinal canal. This chapter provides the anatomic description of these ligaments along with proposed functions and associated clinical implications. It begins with a brief account of the ligaments’ discovery and the various terms used to describe them. The available literature describing the anterior dural ligaments is summarized, highlighting their near ubiquity and highly variable morphology. Three subgroups are detailed: medial, lateral, and radicular. Theories regarding the embryological formation of the ligaments are outlined. The histological structure of the ligaments is explored, as well as how it relates to the proposed functions of the ligaments in stability and protection. We emphasize that the anterior dural ligaments are difficult to visualize on imaging but are important considerations before intervention in the region. Lastly, we provide clinical disorders involving the anterior dural ligaments and expound on areas of current research.
