Radiologic Diagnosis of Cerebral Ischemia: Diagnostics

The field of neurovascular imaging has undergone significant advancements, transforming the diagnosis and management of cerebrovascular diseases, particularly cerebral ischemia. Modern imaging modalities, including computed tomography (CT) and magnetic resonance imaging (MRI), provide detailed anatomical visualization and functional insights, such as hemodynamic measurements. These advances enable clinicians to diagnose cerebral ischemia with greater accuracy and speed, facilitating more efficient treatment planning. This chapter reviews the principles, applications, and limitations of key imaging techniques, including CT angiography (CTA), CT perfusion (CTP), single-photon emission computed tomography (SPECT), magnetic resonance angiography (MRA), MR perfusion and diffusion imaging, vessel wall imaging (VWI), and digital subtraction angiography (DSA). The discussion emphasizes their integration into acute and chronic cerebral ischemia diagnosis, highlighting their clinical utility and emerging technologies like hybrid imaging systems.
