Future Directions and Research in Ischemic Stroke

Stroke remains a predominant contributor to global mortality and morbidity, necessitating advancements in research, therapeutic strategies, and management paradigms. This chapter delineates future directions in stroke care, integrating preclinical and clinical research advancements, telemedicine, rehabilitation innovations, artificial intelligence (AI), and public health interventions to enhance prevention, treatment, and recovery. Efforts to identify and validate biomarkers for stroke subtypes and recovery trajectories are advancing precision medicine approaches. Novel pharmacological and biological therapies, including those targeting inflammation and enhancing endogenous repair mechanisms, are at the forefront of translational research aimed at bridging the gap between laboratory findings and clinical applications. Clinical advancements, driven by innovations in imaging and workflow optimization, aim to maximize therapeutic benefit while navigating the complexities of patient heterogeneity and diminishing effect sizes in broader cohorts. Adaptive trial designs and AI-powered analytics are increasingly employed to address these challenges. Analogously, telemedicine has emerged as a transformative modality in stroke care, enabling equitable access to acute interventions and poststroke rehabilitation. Remote diagnostic and treatment capabilities facilitate timely interventions in resource-limited and geographically dispersed settings. Enhancements in robotics, virtual reality, and neuromodulation techniques represent a promising tool of how to approach neuroplasticity and functional recovery. AI further augments these efforts by delivering predictive models for recovery, optimizing therapeutic regimens, and personalizing care pathways. Last but not least, public awareness campaigns constitute a critical component of stroke prevention and management. Enhanced dissemination of knowledge about risk factors, symptom recognition, and the urgency of treatment through digital platforms and targeted education initiatives aims to reduce delays in care and mitigate socioeconomic disparities.
